Ensure the alignment of technologies, methods and practices with the Home Care Standards, Charter and relevant organisational policies and procedures  Identify individual consumer’s needs resulting in the development and implementation of a holistic therapy plan in consultation with the consumer, carer, relevant stakeholders, and frameworks (internal and external)  Ensure allied health and therapy plans are developed based on the goals, strengths and identified needs in consultation with consumers.
